i'm studyin hard today. gonna learn me some pulmonary system. i already know it pretty well, but i'm gonna get all the details in today. histopath and clinical diagnoses especially. that means i'm gonna be at this damn computer all day. i unplugged the TV. so distract me occasionally with a chat or something occasionally.

up until that last factor, I was gonna hypothesize that skeeter was headin straight for a race... especially with the matching helmet/shirt.... but having seen that bike with no foot retention system, i've gotta invoke the H word. HIPSTER. he's built up an insane, expensive bike, and doesn't know how to ride it. in fact, he probably went with the 650 front for the awesomer barspinz, which, i bet, he can't actually do, since it doesn't look like he has handlebar-toptube clearance. sigh. once you've realized that skeeter's a hipster, it all makes sense; the man-pri short pants; the shoes that match his pants and shirt; the lightening bolt pattern... it's probably a retro T-shirt from Urban Outfitters. my god skeeter. you disappoint me. the only mitigating factor is that he doesn't have a top tube protector on there.

now, there's a SMALL CHANCE, that the shoes he's wearing are clipless compatible, and that his pedals are the platforms w/ clipless portions built in. if that turns out to be the case, i revoke everything negative I said about skeeter, and in fact will hazard a guess that he is actually a tokyo messenger named Skeetaru Yamamoto.

amniotic fluid embolism

Once the fluid is present in the maternal circulation, it progresses to the right side of the heart and into the pulmonary artery and capillaries.

There are a number of theories as to how the amniotic fluid exerts its influence on the circulation. First, it can cause a mechanical obstruction, leading to pulmonary hypertension, decreased left sided filling and consequently systemic hypotension. This would cause an increase in pulmonary capillary wedge pressure, pulmonary artery pressure, and consequently right-sided filling pressure and elevated central venous pressure as was seen initially in our case with the [internal jugular vein ultrasound]. A mechanical obstruction would also cause V/Q mismatch with resultant hypoxaemia. This obstruction to venous drainage could also be the cause of the rapid accumulation of facial oedema, which we saw in our patient. There may also be vaso active components in the amniotic fluid, possibly prostaglandin F2{alpha}, which cause an intense pulmonary vascular constriction. The alveolar capillary leak causing the pulmonary oedema may be a result of extensive microembolic insult, or following a localized or more generalized anaphylactoid reaction mediated by leukotrienes in the amniotic fluid. This could also explain the facial oedema. However, there was no other evidence for an immunologically mediated reaction (rash, bronchospasm, cardiovascular collapse) and plasma tryptase was normal.

i just had a meeting with dr. moncure, chief of trauma : )

we got me set up with all the application stuff for a research position on one project (CRP levels in septic trauma patients receiving sepsis treatment bundles) and in a couple of weeks will be applying for a study on catastrophic brain injury guidelines and treatments.

and "whenever I want to scrub in" with him, i can. oh jesh; that's right.

so, pending approval of my scholarship application(s), i've got an awesome summer deal now.
